INTRODUCTION

Thank you for taking the time to complete your digital health and development questionnaire. The information that you share through this form provides key health information to the HCRG Care Group School Age health service. Completing the questionnaire will help us to understand about your health and wellbeing, and to provide you with information and support around any concerns that you may have. 
A school nurse may ask to see you if you share any concerns about your health and wellbeing; you will be able to share how you would like to be contacted. If you are attending school then teachers and school staff will not be told why we are seeing you.

CONFIDENTIALITY

The information you are sharing today is confidential. This means that any information given will not be shared without your permission to do so. The HCRG Care Group School Nursing Service will not pass on anything you say to anyone else except in exceptional circumstances to protect you or someone else from harm. Anyone who receives information from us is also under a legal duty to keep it confidential. All information that we collect will be stored electronically on secure NHS information systems. All other information collected from you that is not stored on a NHS information system will be anonymised and stored securely on an NHS server for the purpose of public health profiling.
