This Antenatal Health & Wellbeing Contact has been devised by Leicestershire Partnership NHS Trust and the Start for Life Collaboration. Its purpose is to identify any wellbeing concerns that you may have and to offer digital information and support about the topic area that you are concerned about.
It also provides a way to let you know about local services and groups that can support you both during pregnancy and the early months of your baby’s life.
Completing the questionnaire will help us to understand about your wellbeing and also the health of your family. This will enable us to make sure that we offer you the right support and that we have the right services for families in Leicester.
Confidentiality
The information you are completing today is confidential. This means that any information given will not be shared without your permission to do so.
Anyone who receives information from us is also under a legal duty to keep it confidential. The information that we collect will be stored electronically on secure NHS information systems.
Leicestershire Partnership NHS Trust has access to your health records via an electronic database – most GP's and other health services add information to your health records when you go to see them. This can help us to provide the right support for you.
Where you are being seen by other health and support services, we will share information with them where it is relevant and supports your ongoing care and treatment. However, where we asked to share information outside of these circumstances, we will seek your permission to do so.
