INTRODUCTION
When your child first enters school, health advice is transferred from the Health Visiting Service to the School Nursing Service. This service continues until your child leaves education. The School Nursing Team provide an accessible service to children, young people, families, carers and members of the local community.
This digital health and wellbeing form has been devised by School Nurses within the Coventry School Nursing Team. Its purpose is to identify any health concerns that you may have, send you an individualised health care plan for your child and the opportunity to talk to a school nurse should you wish too about your concerns.
Completing the questionnaire will help us to understand about your child's health and also the health of your school community. This will enable us to try to make improvements in the life and wellbeing of young people in your community.
CONFIDENTIALITY
The information you are completing today is confidential. This means that any information given will not be shared without your permission to do so. The Coventry School Nursing Service will not pass on anything you say to anyone else except in exceptional circumstances to protect you, your child or someone else from harm. Anyone who receives information from us is also under a legal duty to keep it confidential. All information that we collect resulting in a contact with our service will be stored electronically on secure NHS information systems.
All other information collected from you that is not stored on a NHS information system will be anonymised and stored securely on a NHS server for the purpose of Public Health profiling. We are required by law to share information to protect you or someone else from harm.
