- It is safe and confidential and designed to help us to help you
- Be open and honest with your responses. This is between you and the School Nurse, who will review your questionnaire

- The School Nurse may want to contact your parents or next of kin to offer support or discuss some of your responses
- We will only talk to someone else if we think you need extra support to keep you safe
- If you are worried about this, please talk to a trusted adult - a parent, relative or teacher
