INTRODUCTION
The school nursing team, along with young people, have created this form for you. This form is for you to be able to tell us of anything you might want help or support with from us or be worried about. When answering the questions on the different topic areas (such as online safety), if you tell us you are worried or want help, then we can meet with you for you to tell us about it. The school will not be told what we are meeting about – this is because it is confidential. That means we will not tell anyone about it. The only time we would have to tell anyone, is if it was something very serious, and meant that you or someone else might be in danger – so we can protect you or them.
By you completing this questionnaire, it helps us to understand and learn more about young people; in your school, and across the city, and how we can help improve yours and their health and wellbeing, so thank you!
