The information that you share throughout this form provides key health information to the Hounslow School Nursing Service. Completing the questionnaire will help us to understand about your health and wellbeing and to provide you with information and support around any concerns that you may have. A school nurse may get in touch if you share any concerns about your health and wellbeing. If we arrange to see you in school, teachers and school staff will not be told why we are seeing you.

The information you are sharing today is confidential. This means that any information given will not be shared without your permission to do so. The Hounslow School Nursing Service will not pass on anything you say to anyone else except in exceptional circumstances to protect you or someone else from harm. Relevant and appropriate information regarding medical needs in school may be shared with the appropriate school staff member. This will ensure your medical needs are supported, and you are kept safe whilst in school. Anyone who receives information from us is also under a legal duty to keep it confidential. All information that we collect will be stored electronically on secure NHS information systems.
Some anonymised data may be shared with other agencies in order to help improve services. All personal information collected from this questionnaire will be seen only by the School Nursing Team. All personal information will be treated as confidential and held securely to meet with data protection requirements.
