INTRODUCTION
This digital health and wellbeing questionnaire has been devised by the Sefton School Nurses. The purpose of the questionnaire is to identify any health concerns that you may have and to offer digital information and support about a variety of topics that may be a concern for you.
If you identify any worries about any of the topics within the questionnaire, a member of the School Health Team may ask to meet you individually at a convenient venue (home, school, health centre). The school will not be told for what reason we are seeing you.
CONFIDENTIALITY
The information you are completing today is confidential. This means we will not share your information without permission to do so. We treat all information with sensitivity. Sefton school health team is part of Mersey Care NHS Trust, which is an NHS service. If we have concerns around you or another person's safety we may need to share this information to keep you safe, this may need to be shared with young people's services. If safe to do so, we will talk to you about how this information will be shared. The school will not be told the reason we are seeing you.
Anyone who receives information from us also has a legal duty to keep it confidential. This electronic form will be stored on a secure NHS information system. This helps us to provide the right support for you. You do not have to share information with other health services, but we do treat all information (where possible) confidentially, this supports us to provide the best care. We are required by law to share information to protect you or someone else from harm.
Responses to some questions may be included in a summary report for other organisations involved in providing healthcare locally, and also your school. However, Mersey Care NHS Trust will remove all personal data and anonymise this before it is shared.
All information that we collect will be stored electronically on secure NHS information systems. All other information collected from you that is not stored on an NHS information system will be anonymised and stored securely for the purpose of public health profiling.
